Formalised knowledge systems, including universities and research institutes, are important for contemporary societies. They are, however, also arguably failing humanity when their impact is measured against the level of progress being made in stimulating the societal changes needed to address challenges like climate change. In this research we used a novel futures-oriented and participatory approach that asked what future envisioned knowledge systems might need to look like and how we might get there. Findings suggest that envisioned future systems will need (...) to be much more collaborative, open, diverse, egalitarian, and able to work with values and systemic issues. They will also need to go beyond producing knowledge about our world to generating wisdom about how to act within it. To get to envisioned systems we will need to rapidly scale methodological innovations, connect innovators, and creatively accelerate learning about working with intractable challenges. We will also need to create new funding schemes, a global knowledge commons, and challenge deeply held assumptions. To genuinely be a creative force in supporting longevity of human and non-human life on our planet, the shift in knowledge systems will probably need to be at the scale of the enlightenment and speed of the scientific and technological revolution accompanying the second World War. This will require bold and strategic action from governments, scientists, civic society and sustained transformational intent. (shrink)

Heraclitean flux plays a large role in Plato 's « Theaetetus » and « Cratylus ». Yet Heraclitus himself did not hold the same conception of flux. The question of how the two thinkers differ, and why Plato treats Heraclitus as he does, is significant because the notion of flux has figured in subsequent philosophical conceptions of the persistence of identity through change. Comparison of Heraclitus, frr. B 12 and B 125 DK reveals that flux is not motion simply, but (...) motion productive of stability ; the unity of stability and flux is the « unapparent connection » that it is Heraclitus ' mission to reveal. Analysis of Tht. 179 C-181 B and Cra. 402 A 3-8 and 411 B-C shows neither any exegesis of Heraclitus ' words, nor any evidence that Plato has in mind any historical Heracliteans. The playful way in which Plato handles Heracliteanism, with comic and hyperbolic imagery, suggests that his audience would have recognized Heraclitean flux as a familiar theory from the context of contemporary oral dialectic. Objectives: This study aimed to determine attitudinal and self reported behavioural variations between medical students in different years to scenarios involving academic misconduct.Design: A cross-sectional study where students were given an anonymous questionnaire that asked about their attitudes to 14 scenarios describing a fictitious student engaging in acts of academic misconduct and asked them to report their own potential behaviour.Setting: Dundee Medical School.Participants: Undergraduate medical students from all five years of the course.Method: Questionnaire survey.Main measurements: Differences in medical students’ attitudes (...) to the 14 scenarios and their reported potential behaviour with regards to the scenarios in each of the years.Results: For most of the scenarios there was no significant difference in the response between the years. Significant differences in the responses were found for some of the scenarios across the years, where a larger proportion of year one students regarded the scenario as wrong and would not engage in the behaviour, compared to other years. These scenarios included forging signatures, resubmitting work already completed for another part of the course, and falsifying patient information.Conclusion: Observed differences between the years for some scenarios may reflect a change in students’ attitudes and behaviour as they progress though the course. The results may be influenced by the educational experience of the students, both in terms of the learning environment and assessment methods used. These differences may draw attention to the potential but unintentional pressures placed on medical students to engage in academic misconduct. The importance of developing strategies to engender appropriate attitudes and behaviours at the undergraduate level must be recognised. (shrink)

Objectives: To explore how subjects in a placebo-controlled vitamin A supplementation trial among Ghanaian women aged 15–45 years perceive the trial and whether they know that not all trial capsules are the same, and to identify factors associated with this knowledge.Methods: 60 semistructured interviews and 12 focus groups were conducted to explore subjects’ perceptions of the trial. Steps were taken to address areas of low comprehension, including retraining fieldworkers. 1971 trial subjects were randomly selected for a survey measuring their knowledge (...) that not all trial capsules are the same. The subjects’ fieldworkers were also interviewed about their characteristics and trial knowledge. Factors associated with knowledge were explored using multi-level modeling.Results: Although subjects knew they were taking part in research, most thought they were receiving an active and beneficial medication. Variables associated with knowledge were education and district of residence. Radio broadcasts benefited those with some schooling. Fieldworkers’ characteristics were not associated with subjects’ knowledge.Conclusions: Research and debate on new or improved consent procedures are urgently required, particularly for subjects with little education. The sequencing of the human genome and decreasing costs of sequencing technology have led to the notion of ‘personalized medicine’. This has been taken by some authors to indicate that personalized medicine will provide individualized treatments solely based on one’s DNA sequence. We argue this is overly optimistic and misconstrues the notion of personalization. Such interpretations fail to account for economic, policy and structural constraints on the delivery of healthcare. Furthermore, notions of individualization based on genomic data potentially take us (...) down the road of genetic reductionism obscuring the role of environmental factors in disease and ill health. We propose that one should see personalized medicine as a way of using personal genomic information to stratify individuals into subpopulations and suggest that personalized medicine be seen within a broader idea of personalized healthcare, reflecting healthcare that integrates personal genomic data into cultural, environmental and personal contexts. Despite Decades of scholarly attention certain sections of Kant's first Critique have proved recalcitrant to received readings, canonical interpretations are impossible to come by. Nowhere is this more apparent than in the literature on Kant's treatment of causality in the Second Analogy, where there exists a controversy of many years standing about the success of Kant's arguments in favour of what has come to be known as ‘the causal principle’. For example, contemporary Kant scholars of stature no less than Lewis (...) White Beck and W.H. Walsh argue that Kant offers a uniquely persuasive case for causation. Other interpreters claim that the materials of the Second Analogy fail utterly to provide what is needed to vindicate judgements of cause and effect in a way that would satisfy the sceptic about causation.2 Thus the despairing conclusion of one recent review of the literature on the Second Analogy:It is possible that there lurks somewhere in the pages of the Critique of Pure Reason a convincing reply to Hume's sceptical doubts about the causal principle. This target article draws together two groups of experimental studies on the control of human movement through peripheral feedback and centrally generated signals of motor commands. First, during natural movement, feedback from muscle, joint, and cutaneous afferents changes; in human subjects these changes have reflex and kinesthetic consequences. Recent psychophysical and microneurographic evidence suggests that joint and even cutaneous afferents may have a proprioceptive role. Second, the role of centrally generated motor commands in the control of normal movements and movements (...) following acute and chronic deafferentation is reviewed. There is increasing evidence that subjects can perceive their motor commands under various conditions, but that this is inadequate for normal movement; deficits in motor performance arise when the reliance on proprioceptive feedback is abolished either experimentally or because of pathology. During natural movement, the CNS appears to have access to functionally useful input from a range of peripheral receptors as well as from internally generated command signals. The unanswered questions that remain suggest a number of avenues for further research. (shrink)

Debate over the status of medicine as an Art or Science continues. The aim of this paper is to discuss the meaning of Art and Science in terms of medicine, and to find out to what extent they have their roots in the field of medical practice. What is analysed is whether medicine is an "art based on science"; or, the "art of medicine" has lost its sheen (what with the rapid advancements of science in course of time, which has (...) made present day medicine more sophisticated). What is also analysed is whether the "science of medicine" is a pure one, or merely applied science; or the element of science in it is full of uncertainty, simply because what is accepted as "scientific" today is discarded by medical practitioners tomorrow in the light of newer evidence. The paper also briefly touches upon how, in the field of present medical education, the introduction of medical humanities or humanistic education has the potential to swing the pendulum of medicine more towards the lost "art of medicine". The paper concludes by saying that the art and science of medicine are complementary. For successful practice, a doctor has to be an artist armed with basic scientific knowledge in medicine. Plutarch's essay de fortuna Romanorum has attracted divergent judgements. Ziegler dismissed it as ‘eine nicht weiter ernst zu nehmende rhetorische Stilübung’. By Flacelière it was hailed as ‘une ébauche de méditation sur le prodigieux destin de Rome’. It is time to consider the work afresh and to discover whether there is common ground between these two views. Rather than offering a general appreciation, my treatment will take the work chapter by chapter, considering points of interest as they arise. This method (...) will enable us to compare what Plutarch says on particular subjects and themes in de fort. Rom. with what he says or does not say about them elsewhere. We shall thus be able to see clearly that for the most part the ideas he presents in the essay correspond with his thoughts about the rôle of fortune expressed in more serious writing, and that, where there is no correspondence, this is attributable to the rhetorical background. I do not intend to address directly the frequently discussed but insoluble question of whether we have in de fort. Rom. only one of two original works, that is whether there was once a de virtute Romanorum which Plutarch composed or answered. De fort. Rom. itself in fact gives almost as much prominence to άρετή as to τúχη, and their competing roles will be carefully evaluated. Nor do I look at the dating of the work. (shrink)

In several US states, the legalisation of euthanasia has become a question for voters to decide in public referenda. This democratic approach in politics is consistent with notions of personal autonomy in medicine, but the right of choice does not mean all choices are morally equal. A presumption against the taking of human life is embedded in the formative moral traditions of society; human life does not have absolute value, but we do and should impose a strict burden of justification (...) for exceptions to the presumption, as exemplified by the moral criteria invoked to justify self-defence, capital punishment, or just war. These criteria can illuminate whether another exception should be carved out for doctor-assisted suicide or active euthanasia. It does not seem, in the United States at any rate, that all possible alternatives to affirm the control and dignity of the dying patient and to relieve pain and suffering, short of taking life, have been exhausted. Moreover, the procedural safeguards built into many proposals for legalised euthanasia would likely be undone by the sorry state of the US health care system, with its lack of universal access to care, chronic cost-containment ills, a litigious climate, and socioeconomic barriers to care. There remains, however, common ground in the quest for humane care of the dying. (shrink)
